It is suggested that you use this tempate to prepare your protocol and then you can cut and paste the various items into your online protocol.  Please keep in mind that some options will only appear in the online protocol as a result of the initial response to the questions. (For example, if you answer no to Is this a multi-site study, the options for outside IRB approval will not appear.  They are shown in this template only to allow you to be prepared to answer those questions if needed.)
