Medical Withdrawal

Medical / Emergency Withdrawal Procedure

1. To withdraw from UW-River Falls, submit a completed Withdrawal Form to the Registrars office. Check "Medical" to request a tuition reimbursement.

2. Follow the Withdrawal Form directions. If you have financial aid, talk with a Financial Aid Counselor. Will you owe money if you withdraw?

3. Call Student Rights and Responsibilities for a Tuition Reimbursement form: 715-425-0720.

4. Submit the Tuition Reimbursement form, with medical/emergency supporting documents, to Student Conduct & Community Standards.

    • Attach personal statement describing the circumstances that led to your withdrawal
    • Attach required supporting documents; any submitted documents are protected by FERPA

5. The Medical/Emergency Withdrawal Committee meets once each month. Afterward, the committee sends you a letter describing its decision.

6. If your tuition reimbursement is approved for more than standard UW System guidelines, you must sign an agreement. This agreement must include documents attesting that the circumstances surrounding your withdrawal no longer exist. Or, you are able to manage them so you can return to UW-River Falls and be successful. Submit the signed agreement and documents to Student Conduct & Community Standards.
