Accident and Injury Reporting

Accident and injury reporting is imperative to maintaining a safe campus.  Reporting any near misses or concerns can aid in prevention of potential injuries. Students, visitors and employees are encouraged to report a safety concern online to the Risk Management Office.

  • As an employee, any accident, injury or work-related illness, should be reported to the employee's supervisor and the appropriate forms completed. This reporting ensures that comprehensive records are maintained and corrective action is initiated to prevent future incidents.
  • A student or volunteer injury should be reported using the General Accident Form. Send the form to Risk Management 25H North Hall.
  • Accidents involving a fleet vehicle require completion of the Vehicle Accident/Incident Report.

If you are an employee injured on campus, it is imperative you take the steps given below. Your health, future well being and worker's compensation coverage greatly depend upon your taking correct and immediate action.

  1. Seek medical attention or first aid as quickly as possible, informing your provider you have a worker's compensation claim. 
  2. Within 24 hours of the injury, report the occurrence to your supervisor and complete the Employee's Work Injury and Illness Report.
  3. In addition to the Employee's Work Injury and Illness Report, two more reports must be completed by you and your supervisor immediately.  The forms can be found on the Worker's Compensation webpage. For more information regarding these forms, email hr@uwrf.edu

If you are a student or visitor of UW-River Falls and are injured on campus, seek immediate medical attention as needed and complete a General Accident Report.  Submit the completed report risk@uwrf.edu or call 715-425-3344 with any questions.

The university does not provide any type of compensation for injuries that occur on campus.  Students and visitors are expected to maintain their own health insurance to cover medical expenses.

Volunteers On Campus

A volunteer is a person who provides services freely and without coercion, direct or implied; who neither receives nor expects to receive any kind of pay or compensation for his or her services; and, are not otherwise employed by the same public agency to perform the same services as those for which they propose to volunteer.

Volunteers should not perform duties that are typically performed by permanent employees. Volunteers should be supplementing the work done by employees, not replacing the need for paid staff.

PLEASE NOTE:  All requests must be submitted by an employee of UWRF. This should not be used for anyone who will be entered into the HRS system as an employee of UWRF or anyone currently enrolled as a student.

Volunteer requests provide the detail that is needed for an individual on campus serving in any non-employee role; also for granting access to campus resources for individuals who do not have UWRF HR appointments.  (Examples include but are not limited to:  visiting scholars, student org advisors, interns, camps/clinics facilitators, University Advancement employees, etc.) .

All volunteers are required to complete Executive Order #54 Required Training for Volunteers. If identified as a Position of Trust, a criminal background check is also required prior to appointment.


Non-discrimination Clause

Non-discrimination/Affirmative Action: State contracts must contain a provision prohibiting the contractor from discriminating against its employees or applicants for employment on the basis of age, race, religion, color, handicap, sex, physical condition, developmental disability, sexual orientation, or national origin. Contractors of a certain size are required to have an affirmative action plan. See your grants or contracts specialist if you have any questions. Wis.
