Driver's Authorizations

Any person driving a state-owned vehicle or their personal vehicle on state business must read the State of Wisconsin Fleet Policies and Procedures Manual.

Processing time is ten business days.

If you have an out of state drivers license you must complete the second page of the form and have it notarized. Notary Publics on campus are located at: 136 Kleinpell Fine Arts, 205 Wyman Education Building, 310 South Hall, 8 & 111 North Hall, 170 University Center, 132 Central Stores.

Complete the Drivers Authorization form if you...

• Are a New Employee
• Are a student. The approval is valid for 12 months.
• Have a change of driver's information. (state, license, etc.)
• Have an out of State Drivers License.The approval is valid for 12 months.

You do not need to complete a new Drivers Authorization form if you...

• Have a Wisconsin driver's license, do not have a change of driver's information, and have completed the Drivers Authorization after 1/1/06. The approval is valid unless violation(s) have occured.
