Employee Assistance Program

The Employee Assistance Program (EAP) is a referral service for employees and their families who are looking for assistance to address mental health and/or chemical health concerns. Faculty/Staff can meet with a counselor for a one-time referral session to assess needs and discuss appropriate resources. Simply call 425-3884 for an appointment to meet with a professional counselor.

What to Expect From Counseling

You may print and complete the following forms and bring them with you to your first appointment:

• EAP Counseling Intake Form 
• Informed Consent Form print extra copy to keep for yourself, and bring signed copy to your counselor

You may also come in twenty minutes before your first scheduled appointment to complete your paperwork if you wish.
