We value and appreciate any compliments, suggestions, information, and/or concerns that you can provide about the Office of Undergraduate Research, Scholarly and Creative Activity, its staff, and its programming.  Please follow the appropriate steps below to assist us as we work to serve the university and its students well.

Compliments, Suggestions, or Information

If you have compliments, suggestions, or information that you would like to share about any of the staff reporting to the Office of Undergraduate Research, Scholarly and Creative Activity, please provide the following details:

• Name or description of the person(s)
• Date
• Location, if applicable
• Your contact information (this is optional): name, email, and cell-phone
• A complete description of the compliment, suggestion, and/or information that you would like to share.

To submit this in a non-confidential manner, please email the information.

To submit this confidentially, please mail the information without any identifying information to Dr. Earl Blodgett, UW-River Falls, 410 S. 3rd St., 235 Rodli Hall, River Falls, WI  54022 (you are welcome to use campus mail where practical).

If you have compliments, suggestions, or information that you would like to share about the Director, Dr. Earl Blodgett, please provide the information above and for non-confidential sharing, email the information.  For confidential information, please mail the information without any identifying information to Associate Vice Chancellor, Kathleen Hunzer, UW-River Falls, 420 S. 3rd St., 213a Rodli Hall, River Falls, WI  54022.

Concerns or Complaints

We endeavor to provide excellent service.  However, if you have concerns or complaints about any of the services provided by the Office of Undergraduate Research, Scholarly and Creative Activity, please take the following steps to resolve the issue:

1. Discuss the issue with the individual involved.  If a satisfactory outcome does not occur, proceed to step 2.
2. Discuss the issue with individual's supervisor.  If a satisfactory outcome does not occur, proceed to step 3.
3. Continue with this process and if a satisfactory outcome does not occur, discuss the issue with the URSCA Director, Dr. Earl Blodgett.  If a satisfactory outcome does not occur, proceed to step 4.
4. Discuss the issue with the Associate Vice Chancellor for Academic Excellence and Success.  You will need to schedule a meeting by calling (715) 425-0720.  If a satisfactory outcome does not occur, proceed to step 5.
5. File a formal complaint following the directions here.
