Reporting Concerns

UW-River Falls values a diverse community where all students, faculty, and staff are able to fully participate in the campus community. We encourage reporting so that we can appropriately and promptly address any concerns. Use this form if you are concerned about the actions or the welfare of a student. Report a Concern

Sexual Misconduct

Use these forms to report any form of sex-based discrimination or misconduct (i.e., sexual assault, sexual harassment, dating or domestic violence, stalking, or sexual exploitation). More information can be found on the Title IX website.

All university employees, must report concerns about sexual assault, sexual harassment, dating or domestic violence, stalking, or sexual exploitation. Report a concern if it involves a student, regardless of where it occurs, and report any concerns on, or adjacent to, campus or in a university program or activity, regardless of who it involves. Mandated Reporting of Sexual Misconduct

Students should use this form to report a concern to the university, when the concern relates to sexual assault, sexual harassment, dating or domestic violence, stalking, or sexual exploitation. Information entered here will be reviewed by the Title IX Coordinator and may lead to an investigation by the university and appropriate follow-up action.
