Insurance
Students are strongly encouraged to carry
health insurance, since Student Health Services is NOT health insurance.
If students are currently covered by
health insurance, they should check with the carrier to see what
arrangements need to be made so coverage continues while at college. All students should carry health insurance cards with
them. Participation in the University of Wisconsin System Student Health Insurance Plan (see below) is required for international students.
Additional coverage must be purchased for Varsity Athletic coverage, for both domestic and international students. Please see below for additional information.

2009/2010 Plan Provided Through Student Assurance Services:

Must enroll online (domestic students) or send in via regular mail (for additional athletic coverage). Cannot enroll by faxing or calling. Coverage becomes effective on the later of: the Policy Effective Date (8/05/09); the first day of the term for which the proper premium has been paid; or 12:01 a.m. following the date the proper premium is received by Student Assurance Services. All coverage expires on 8/04/10, or when payment is due and unpaid. It is your responsibility to make timely premium payments regardless of whether or not you receive a premium notice.
Please note that for domestic students, this policy has a $100,000 per injury or sickness maximum, limits inpatient hospital care to $1,200 per day, but does have a $5,000 for PPO providers and $10,000 for non-PPO providers limit on out-of-pocket expenses for the student.

6. If a provider is listed, follow up by calling the provider to confirm that they are still affiliated with HealthEOS.
