Athletic Training

The UW-River Falls athletic training staff provides on-site medical coverage for 18 varsity teams. Services provided by the athletic training staff include prevention, evaluation, treatment, and rehabilitation of athletic injuries.

The UWRF athletic training staff works in conjunction with physicians from St. Croix Orthopedics, Hennepin County Medical Center and the River Falls Area Hospital and Clinic.

Student Athlete Forms

The following four forms should be printed, filled out completly and returned to the appropriate personnel by dates indicated. No UWRF student-athlete will be able to practice until ALL forms are filled out and received. Every student-athlete must also submit a copy of the front and back of their current insurance card.

• General Information Form
• Acknowledgement of Insurance Requirements Form
