Worker's CompensationThese forms need to filled and returned to the Human Resources Office within 24 - 48 hours of the injury/accident.UNIVERSITY OF WISCONSIN – RIVER FALLSINSTRUCTIONS FOR COMPLETING INJURY REPORTThere are mandatory forms that must be completed when reporting an employee injury. The instructions for completing the forms are outlined below. Because we must process all injury reports in a timely manner, it is important that they be filled out completely and accurately. If you have any questions about these forms, please call Tracey Nyeggen at the Office of Human Resources at 715-425-3382 or tracey.nyeggen@uwrf.edu. INJURED EMPLOYEE: (two forms for employee to complete) These forms need to be to Human Resouces within 24 hours of the injury/accident. Complete section 1 on the Employer's First Report of Injury or Disease. In addition, compete all of the Employee's Work Injury and Illness Report. SUPERVISOR: (two forms for supervisor to compete) This form needs to be to Human Resources within 48 hours of being reported. The Supervisor needs to complete section 4 of the Employer's First Report of Injury or Disease after the employee has filled out Section 1. The Supervisor must also fill out the Supervisor's Accident Analysis Prevention Report (2 pages). If the injury also involves repetitive motion or material handling, complete the Repetitive Motion and/or Prevention Report (page 2 of Supervisor's Accident Analysis Prevention Report) .
