Office of Human Resources · 216 North Hall · (715) 425-3221 · Fax 425-3365 · www.uwrf.edu/hr/ DATE: April 11, 2008 TO: Supervisors of Classified Employees It is the time of the year for supervisors to conduct annual performance evaluations for classified staff who are not on probation or on a leave of absence (if an employee is currently on probation it is necessary to conduct a three-month and five-month evaluation from the date of hire). Though you should be having discussions with the employee throughout the year regarding his/her performance, it is especially important that you take this time each year to formally evaluate the employee. This is a key responsibility of each and every supervisor on campus. Currently, you should have an evaluation form in your office for each employee for which Section 1 (Key Responsibilities) and Section 2 (Expected Results) were completed last April. This would have been considered the planning session. The following steps should now be taken: Document actual results by completing sections 3 through 7 of the Classified Employee Performance and Development Review ( http://www.uwrf.edu/hr/hr_forms/Classified/cl_evaluation_form.doc ). Schedule a time to discuss the Performance Evaluation with the employee. (Keep in mind that employee input is an important part of the performance standard process; you may want to ask the employee to do a self-appraisal as part of the evaluation procedure.) After your discussion, sign Section 7. The employee must also sign the evaluation. (As indicated on the evaluation, the employee's signature does not indicate agreement but attests that the employee had an opportunity to read and discuss the review .) In addition, the employee may want to include comments or identify career goals. This can be done in Section 5. Make two copies of the completed form; keep one copy for yourself and give one to the employee. Please forward the original form to my attention in the Human Resources Office by May 21, 2008. The original will be placed in the employee's personnel file. Please also review the position description for possible updates. If changes occur, please send the updated position description along with Page 1 of a completed Position Description Form ( http://oser.state.wi.us/docview.asp?docid=1222 ) to Human Resources. If you intend to administer a disciplinary warning at the performance evaluation session, please notify the employee of your intent and the employee's right to representation.
