This form must be completed prior to registering for HEAL 470, P ED 271, 371, or 471. Bring completed materials to the appropriate instructor to obtain an add card to register for class.

Projected Schedule Planner
A form to help you plan your class schedules. Several courses have pre-requisites and not every course is offered each semester. It is critical that you plan for more than one semester at a time.

Substitution Request
All substitution requests must be approved by the Department Chair. Call the main office at 425-3705 to schedule an appointment. Bring a current DARS with to the appointment with you.
