If you are applying for a PLUS Loan program for your son/daughter attending the University of Wisconsin - River Falls please complete the form below and submit it to the Office of Financial Assistance by clicking on the SUBMIT button below.

By submitting this PLUS Loan Request form and completing the eMPN (next step) it is assumed that you authorize the University of Wisconsin - River Falls to directly credit the PLUS loan proceeds to your student’s account and have any unused proceeds refunded directly to the PLUS borrower of record. If you agree to this assumption check this box: I agree.

If you don't agree to the direct credit of your PLUS loan proceeds a paper check requiring your endorsement will be requested from the lender. This may increase the time to process your loan proceeds.
