Final Student Report (Required before credits will be awarded)

This is to be a formal report of your entire Internship experience. The report should include a cover page and the following:

Site Information

• Name and address of the site
• Name, phone number, and email of supervisor

Job Information

• Dates of the Internship
• Job Title
• Dob Description
• Major Resposnsibilites
• Specific projects you completed or assisted in. You may include print-out samples

Internship Evaluation

• Overall evaluation - was it worthwhile? Did you have any problems? What you have learned? Any other positive or negative aspects of the internship?
• Suggestions that you have on how the Internship could have improved.
• Suggestions that you may have for improving the CIS curriculum based on your internship experience
• Any other comments

Reflective Essay

• Describe in an essay or a reflective moment when you realized you have just learned something important about being a computer professional, working as a member of a group, or working in a tech organization.
