Have official TOEFL test result sent to the address below.
For TOEFL information visit ETS.org. The UWRF school code is 1918.

Complete and sign disclosure of ethnic/racial information form

Complete the Report of Financial resources form (.pdf)

Provide all requested information
Pledge to the accuracy of the information before an official who will witness the pledge (U.S. Government official or Notary Public or Commissioner for Oaths or other public official with authority to validate oaths.
The witness to the pledge of accuracy must sign and describe their position of authority.

Mail all materials and $56 USD application fee payable to UW-River Falls and official English language copy of transcripts to:
