This application is only one of four pieces of information that must be turned in before your internship can be approved and processed. In addition to this application, you must manually submit:

A job description from the employer that includes the tasks and projects to be worked on during the internship and the intern's work schedule.
A current copy of your Degree Audit Report (DAR).
A cover letter to the Department Chair noting how your internship relates to your career interests and major.
