UWRF FoundationAccount Management FormsPlease recycle and stop use of any former versions of these forms. These forms have been revised. There are two versions for each of the newly designed forms: the Online version is to be completed on your computer and the Printable version is to be printed then completed by hand.
Deposit Form: Fund managers should use this form when they need to deposit money into their UWRF Foundation account. Request for Payment Form: Use this form when you need to have money paid from your UWRF Foundation account. Examples include: payment that needs to be made to a university account, an outside vendor or individual. Request for Transfer Form: Use this when you need to transfer funds from one UWRF Foundation account to another UWRF Foundation account. An example would be transferring money from your Foundation department account to a Foundation scholarship account. New Account Request Form: Use this form when you are interested in opening a new UWRF Foundation account. (Online) (Printable) Request to Close Account Form: Use this form when you are interested in closing a UWRF Foundation account. (Online) (Printable) Mileage Reimbursement Form: Use this form when reimbursement of mileage is needed from a UWRF Foundation account for university or foundation related business.
