Incident Mandated Reporting Form

Wisconsin Statute 36.11 (22) requires that any university employee, including some student employees, i.e., resident assistants and hall managers, who either (a) witness a sexual assault involving a student or (b) receives a report from a student who has been sexually assaulted, must report the incident to the Director of Student Conduct & Community Standards: 715-425-4844.

UW-River Falls reports its sexual assault statistical data annually to UW System. UW System then summarizes all system campus data and forwards it to the Wisconsin Department of Justice (DOJ).

The Campus Community Sexual Assault Coalition has compiled a Campus and Community Sexual Assault Resource and Services Guide.

If you receive a report about a sexual assault, please know that the campus and community have victim support resources.
