Incident Mandated Reporting Form

Wisconsin Statute 36.11 (22) requires university employees, including some student employees, who:

  • witness a sexual assault involving a student or 
  • receive a report from a student who has been sexually assaulted, 

to report the incident to Gregg Heinselman, Title IX Coordinator, and Assistant Chancellor for Student Affairs, 715-425-0720.

UW-River Falls reports its sexual assault statistical data annually to UW System. UW System then summarizes all System campus data, and forwards it to the Wisconsin Department of Justice (DOJ).

The Campus Community Sexual Assault Coalition has compiled a Campus and Community Sexual Assault Resource and Services Guide.

If you receive a report about a sexual assault, the campus and community have survivor support resources.

In addition to recourse and support through the City of River Falls Police Department (9-911), on-campus help is available through:
