The University of Wisconsin River Falls encourages students, faculty, staff, and community members to report concerns regarding the welfare and safety of our students whenever concerns arise. The primary goal of the University is to promote a culture of reporting that aids in assisting students, addressing safety concerns, and addressing issues that are affecting the UWRF community.
The Report a Concern form can be used
to report general concerning behaviors of students that you would like the
University to know about. This could be regarding student conduct, mental
health, physical health, or any other general concerns that you have about
a student or the UWRF Community. To complete this form simply click on the "Report a Concern" button above and complete the web based form.
In Accordance with Wisconsin Statute 36.11 (22) all University employees, including some student employees (i.e., resident assistants and hall managers) who either:
- witness a sexual assault involving a student or
- receives a report from a student who has been sexually assaulted,
must report the incident via the Mandated Reporting Form to Gregg Heinselman, Title IX Coordinator and Associate Vice Chancellor for Student Affairs. Additionally, students wishing to report a sexual assault, or other sexually based incident to the University should use the Mandated Reporting form to file a report.
To complete this form click on the "Mandated Reporter Form" button above. The form should be printed, filled out completely, and returned to the office of Gregg Heinselman, Title IX Coordinator
and Associate Vice Chancellor for Student Affairs, located in the University Center 170.
