Any person driving a state-owned vehicle, a rental vehicle, or their personal vehicle for university purposes must read the State of Wisconsin's Fleet Driver and Management Policies and Procedures Manual. For information regarding the use of your personal vehicle for university purposes click here.
A Driver Authorization Form must be completed and turned in or mailed to the UWRF Risk Management Office for approval. Processing time is one week.
Are a new employee. (The approval is valid for your employment period at UWRF.)
Are a student or an agent of UWRF. (The approval is valid for 12 months and expires one year from the application approval date.)
Have a change of driver's information. (e.g., state, license, name change, etc.)

Must have a valid drivers license (new residents to Wisconsin must obtain a drivers license within 30 days of residency).
Must have a minimum of two years licensed driving experience.
Must be at least 18 years of age.
Three or more moving violations and/or at-fault accidents in the past two years.
An OWI or DUI violation within the past year.
License has been reinstated for less than one year after suspension/revocation.
Please note: Employees, LTEs, students, and agents will have their driving records checked periodically. Authorization ends when the driver fails to meet the minimum driving standards, the approval expires, or employment is terminated.
With the Nationwide Vehicle Rental Services contract, 15 passenger vans will still be available for rental. There are restrictions with this availability. Please see the "Contract User Guide" available at the following link:

The driver(s) must have successfully completed the 12 and 15 passenger van driver training program, be current in their frequency of use of these vehicles in order to meet the state's 12 and 15 passenger van driver training requirements and must comply with 12/15 Passenger Vans Driver, Training and Usage Guidelines. (Frequency of use requires that a driver must have driven a 12/15 passenger van a minimum of 10 hours or 400 miles in prior three months). In addition, UW System policy requires students to be 25 years of age in order to drive a 12 or 15 passenger van.
Rental of a 12 or 15 passenger van must have pre-approval for each rental from the Department of Administration Bureau of State Risk Management. At UWRF, this will be coordinated through Connie Smith in the Risk Management Office. Please contact Connie if you have the need to rent a 15 passenger van through the state rental service contract. She will work with DOA to obtain the approval on your behalf. 15 Passenger Van driver training will still be coordinated through Facilities Management.
The Department of Administration will monitor the usage of 12 and 15 passenger vans. Drivers who rent 12 and 15 passenger vans without prior approval may have their van driver training certification revoked as well as the approval to rent or drive other state vehicles.
