A damaged or lost item may qualify for replacement by you. In lieu of paying the standard $110 lost/damaged item fees, you may provide a specified replacement and pay a $25 processing fee. Reserve items and items that have already been replaced by the library are not eligible for replacement by the user.
Here is how the process works. Submit this request form and a library staff member will provide you with details on what to order and where to ship the material. Once the material arrives at the library, a staff member will review the copy supply and accept or reject it. (If you ordered the specified item the library will accept it.) The $110 fee will be removed from your account. After you have paid the $25.00 processing free your account will be cleared.
