New Employee Forms

The forms listed below are required in the Office of Human Resources, 216 North Hall on your first day of employment. If you have questions concerning these forms, please contact your Human Resources representative.

I-9 Employment Eligibility Verification form and instructions  The I-9 form must be completed on your first day of employment. The link includes a list of acceptable documents which can be used to establish identity and work authorization.

Emergency Contact Information Form - Information from this form will be used in the event of an emergency.

W-4 Federal and State Employee's Withholding Certificate

Veterans New Hire Form, Employee Disability Self-Identification Survey - Voluntary disclosure, complete entire form.

What is an I-9 Employment Eligibility Verification form?

An I-9 Form is the Employment Eligibility Verification Form required by the Immigration and Naturalization Services (INS) to verify your identity and your eligibility to work. All employees must complete this form and provide valid original identifications. You are not eligible for pay until Payroll Services receives a satisfactory I-9. 

How do I complete the I-9 form? 

Fill out and sign Section 1 of the I-9 Form on your first day of employment. By the 3rd day, submit it with valid identification documents to your Home Department Coordinator, who will complete the rest of the form. Valid documents for proof of identification and of employment eligibility are categorized into 3 lists. You may submit originals of either:
    •    one document from List A,   or
    •    one document from List B   and   one document from List C.
