Self-disclosing to seek an option for accommodations.

College students have a strong right to privacy, so to start the formal process they must outwardly request this kind of attention. It has to be either the students or a legally authorized representative who makes the request (family members and friends rarely have that legal status). 

A convenient way to submit the request at UWRF is with a Self-Disclosure Form that is available from its Disability Services Office.
