Tips for completing your application

- A verification of completion certificate will be emailed to you upon endorsement of your license. This can be used as evidence your license is in process with the Department of Public Instruction

- Candidate fails to have application notarized when signing it
- Graduate students or Non-degree seeking students do not list all schools and degrees under the education area

Tips for completing your application
- Read all directions carefully before beginning the online application
- You only need to submit the MN verification of program completion to me to complete. I will return this to you to submit with the rest of the required materials.
- Official Transcripts can be ordered through the Registrars office online for $8.00. If you rush order them or pick them up in person there will be an additional $8.00 fee.
- MN requires all supplemental materials to be submitted in a single packet
- Make a copy for the verification for your records
- You can e-mail me the verification form as a .pdf attachment, However I have to send it back via the Postal Service
- With your verification we will include a letter which will provide MN with your praxis testing information. DO NOT OPEN THIS LETTER! My signature will be across the envelope to show I sealed it.

Questions about Praxis Testing and Minnesota
- MN will not accept any Praxis exams completed after 9/1/2010
- MN will only accept Praxis I and II exams they required previously until 9/1/2012 After that date you will need to complete the MTLE testing requirements (an example of this is MN will not accepted the Middle School Content Knowledge exam 0146 or 5146 because they never accepted it.)
- MN will only accept an original score result or a sealed letter from UWRF sharing your test results.
